FINANCIAL & APPOINTMENT POLICY

When scheduling an intake appointment or consultation you are asked to pay in advance as a scheduling deposit. We cannot hold your appointment until you have paid this deposit. Our intention is to encourage those who have a scheduled appointment to honor their commitment to the process, as no-shows limit other patients’ access to medical care.  This scheduling deposit is fully refundable, if you cancel at least 48 business hours before your appointment. For example, if you have a 1:00 PM appointment on a Monday, the deadline for cancellation or rescheduling would be the prior Thursday at 1:00 PM.

This deposit will be forfeited if you are more than 20 minutes late, or cancel with less than 2 business days’ (48 hours) notice.  Please note you may always cancel your appointments with us at any time; however, you may be charged as described above if you do so without adequate notice.

Boulder Mind Care does not accept insurance, Medicare, or Medicaid for ketamine infusion therapy. Currently, we accept cash, ACH transfer, credit cards, debit cards, HSA accounts, and Care Credit. With Care Credit, you can spread out the payments for the Initial Consult and Initial Series of six infusions so that it is much more affordable. We require payment in full before treatment, at least two business days in advance. Payments are processed by a secure and encrypted payment processor.  You also will be asked to provide a credit or debit card, which will be retained for future services.

We realize there are other clinics that give cheaper ketamine therapy. Some of them adopt a ‘Dollar Store’ approach. Some do not give intravenous ketamine, which is the “Gold Standard” and the most precise, controllable, and effective route of administration. Some treat multiple patients at the same time in the same room. Some monitor their patients infrequently, not at all, or using inexperienced medical assistants. We strive to provide you with a private, serene environment. We are dedicated to supporting you to live the life you desire. We do not overbook or rush our sessions with you. Our fee schedule is as follows:

Initial Consultation: $390 (due at time of scheduling the appointment).

Infusions for mood disorders: $690 per infusion (due in full two business days before date of infusion appointment). These are between 40 and 55 minutes duration.

Infusions for chronic pain: $1,500 per infusion (due in full two business days before date of infusion appointment). These are typically 2 to 4 hour infusions, most often done between 3 and 5 days in a row.

Infusions for drug and alcohol addiction: $690 per infusion (due in full two business days before date of infusion appointment). These are between 40 and 55 minutes duration.

Infusions for KAP (Ketamine-Assisted Psychotherapy): same as above for mood disorders less than one hour duration; $900 for 90 minute sessions, $1,150 for 2 hour sessions.

We offer a 10% discount to veterans, active duty military, retired and active duty law enforcement, other first responders, and other licensed medical professionals. Thank you for your service!

For those with PPO type health insurance, you may be able to get partially reimbursed for the cost of your visit, though this is not guaranteed. After you complete your initial series of infusions, you may request a receipt (superbill) from us which details procedure codes, diagnosis, and fees paid. You may then submit the superbill to your insurance provider for direct reimbursement to you. We do not process insurance claims or communicate directly with insurance companies.

Finally, Medicare patients will be asked to sign an advance beneficiary notice acknowledging that we do not accept Medicare.
